โรงพยาบาลพระปิยมหาราช โดยกลุ่มงานเภสัชกรรม มีความจำเป็นต้องจัดซื้อยา Denosumab 50 mg Injection จำนวน 80 กล่อง (กล่องละ 3 แผ่น) ด้วยเงินบำรุงโรงพยาบาลพระปิยมหาราช ประจำปีงบประมาณ 2569 วงเงินงบประมาณประมาณ 876,624.00 บาท โดยมีวัตถุประสงค์เพื่อจ่ายยาให้ผู้ป่วยที่มารับการรักษาในโรงพยาบาล

คุณลักษณะยาที่ต้องการเป็นยาฉีดปราศจากเชื้อสำหรับฉีดเข้าใต้ผิวหนัง (Subcutaneous Injection) ความแรง 50 mg/ml ปริมาณ 1 มิลลิลิตร บรรจุในเข็มฉีดยาพร้อมใช้ (Prefilled Syringe) ผลิตภัณฑ์ต้องผ่านการตรวจวิเคราะห์คุณภาพครบทุกรายการ ได้แก่ Appearance, Identification, Purity & Impurities (SE-UHPLC, Reduced CE-SDS, Cation Exchange HPLC), Sterility Test, Bacterial Endotoxins, Protein Concentration (54.0–66.0 mg/ml), pH (5.0–5.5), Osmolality, Volume และ Subvisible Particulates ตาม Finished Product Specification ที่จดทะเบียน

ผู้ขายต้องส่งมอบพัสดุภายใน 365 วันนับจากวันลงนามสัญญา ภายใต้รูปแบบสัญญาจะซื้อจะขายแบบราคาคงที่ไม่จำกัดปริมาณ โดยส่งมอบตามใบสั่งซื้อแต่ละคราวภายใน 14–15 วัน ยาที่ส่งมอบต้องมีอายุคงเหลือไม่น้อยกว่า 1 ปี รับประกันความชำรุดบกพร่องอย่างน้อย 1 ปี และต้องจัดส่งด้วยระบบ Cold Chain (2–8 องศาเซลเซียส) ที่ได้มาตรฐาน พร้อมรับเปลี่ยนยาคืนทุกกรณีที่ยาใกล้หมดอายุหรือเสื่อมสภาพโดยไม่มีเงื่อนไข

เงื่อนไขสำคัญ ผู้เสนอราคาต้องแนบเอกสารขึ้นทะเบียนตำรับยา (ทย.1–4, ย.1–2) ใบรับรองมาตรฐานการผลิต GMP ตามหลักเกณฑ์ PIC/S เอกสารคุณภาพยา (API/Finished Product Specification, CoA, ผลการศึกษาความคงตัวระยะยาว) และเอกสารรับรองรุ่นการผลิต (Lot Release) จากกรมวิทยาศาสตร์การแพทย์ เนื่องจากเป็นยาชีววัตถุ การพิจารณาคัดเลือกใช้เกณฑ์ราคาโดยพิจารณาจากราคารวม และกำหนดอัตราค่าปรับกรณีส่งมอบล่าช้าที่ร้อยละ 0.02 ต่อวันของราคาสิ่งของที่ยังไม่ได้รับมอบ
